Complete this form to begin your PRO Baseball Path journey. All information is kept strictly confidential and used only for program development and communication.

For a participant under 18, this form is completed by the natural guardian. A participant who is 18 or older completes it for themself.

Medical and injury information is collected on this form only. PRO stores it as a sealed record, uses it for safety and planning, and never shares it without your written request.

PAR-Q+. The Physical Activity Readiness Questionnaire is a separate sealed document that PRO provides. It is not completed on this site. PRO stores only whether a PAR-Q+ is on file and its date.

Electronic Signature

The parties agree to conduct this transaction electronically. A signature entered on this page, consisting of the signer's typed name, role, and date, together with the submission timestamp and network address recorded by the form service and the SHA-256 hash of this document as displayed, constitutes a valid signature and record under the federal ESIGN Act and Florida's Uniform Electronic Transaction Act. PRO retains the signed record and will provide a copy on request. A signer may instead sign on paper.
