Signing Packet

OB Personal Management, LLC d/b/a PRO Baseball Path. PRO Venture Group Inc. owns the PRO platform and methodology and is an additional Released Party.

What you are signing

1. Player Intake - contact, emergency, and medical information. Completed by the parent or guardian for a participant under 18.
2. Assumption of Risk, Release, and Medical Authorization - the waiver, including the media election and the Florida notice for guardians of minors.
3. Motion Capture, Biometric Data, and Data Use Consent - a separate, voluntary consent. You may decline it and still continue.
4. Client Services Agreement - the plan you are choosing, billing, cancellation, and terms.

What to have ready

• The participant's date of birth, school, team, and positions.
• Home address, parent or guardian contact details, an emergency contact, and any injuries or medical conditions PRO should know about.
• The plan you discussed with PRO and the start date you agreed.

Who signs

For a participant under 18, the natural guardian signs every document. A participant who is 14 or older also signs the waiver and the consent. A participant who is 18 or older signs for themself.
